Download, fill out, and print the form to request items like chairs, sofas, beds (twin, full, queen), mattresses, or clothing (subject to availability). Key details include:

• Provide client information: full name, date of birth, phone number, and current address/shelter location.
• Specify current living situation: transitional housing, recently housed, or emergency shelter.
• Indicate preferred pickup or delivery date and client-available time window.
• Describe the client’s need and how furniture supports their transition to sustainable living.
• Include referral agency representative details for Embrace Hope / C Carter Crane.
• The partnership aims to offer hope, comfort, and a fresh start with the motto "Together, we are better."
